The UW Medicine Heart Institute at Harborview has an outstanding opportunity for a highly motivated PROGRAM COORDINATOR (Full-Time, Day Shifts) to help facilitate care for our patients. This position will work with the Heart Institute Clinic and Community Heart Failure Program teams.

The Community Heart Failure & Heart Institute Clinic Program Coordinator is responsible for coordinating the care and disposition of patients who require urgent pre-operative, diagnostic, and routine care. Broadly, this includes interfacing with insurance companies to determine coverage, scheduling clinic appointments, obtaining medical records prior to clinic appointments, and scheduling clinic visits. Additionally, the Program Coordinator will serve as the primary contact between Harborview and the referral community, as well as provide support for the providers in clinic.

MAJOR RESPONSIBILITIES:

  • Serve as primary program contact for other departments, patients, residents, fellows, other hospitals, outside agencies, and general public; provide information regarding program policies and services.


  • Manage provider templates and schedules.


  • Ensure completeness and accuracy of registration and patient demographic data.


  • Obtain all pertinent medical history and outside records.


  • Maintain an organizational system to track multiple referral streams (hospital follow-up, consultations, procedures, radiologic studies) and track patient records.


  • Coordinate complex scheduling of diagnostic procedures and consultations.


  • Facilitate scheduling of urgent referrals and procedures, working closely with Clinic medical director and nursing staff to triage.


  • Manage a high volume of phone calls, emails, faxes, and letter correspondence pertaining to patient and/or provider follow-up, continuity of care, and referral decisions.


  • Communicate with discharge planner and Contact Center Complex Appointing Team to arrange for timely post-discharge care.


  • Interface with UW Medicine Contact Center and maintain scheduling procedures consistent with clinic and patient access goals.


  • Utilize system resources, including computerized information systems and staff from ancillary departments, and develop processes to meet the needs of the service and patients.


  • Independently prioritize patient scheduling and follow-up needs. Follow through and resolve patient requests on a timely basis.


  • Triage patient phone calls according to severity of need; follow through and resolve patient requests in a timely manner


  • ADDITIONAL DUTIES:

  • Assist management with operational process improvement. Provide feedback and make recommendations for streamlining department objectives.


  • Cover for non-clinical staff absences as necessary to maintain clinic operations.


  • Create ad hoc data reports to identify clinic trends and track incoming referrals and patient show rates in order to assist clinic directors in planning future clinic growth.


  • Assist with new staff orientation, train new PC and PSS staff.

  • REQUIRED QUALIFICATIONS:

    High school graduation or equivalent AND two years of experience in the program specialty

    OR

    equivalent education/experience.
